How to Create a Web Form with Zapier and Connect it to

Our Zapier integration helps business owners save time by automating processes that are typically done manually.

This guide will walk you through how to create a web form with Zapier.

In doing so, leads generated through your Zapier form can be sent directly to's Outreach Campaigns team for outbound callback.


To connect a Zapier form to, you will need:

Request an Outbound Call from a Zapier Form

  1. Sign up for a Zapier account. If you already have one, log in.
  2. Enable "Zapier Interfaces" for your account by visiting this page. Or, in your account, you can toggle there in the left hand drop-down menu.
  3. Click on the button "Create your first interface."
  4. There are many options that you can select. Click the option for "Form."
  5. You will need to now create Pages where your form will exist. Typically, you will have two pages, at minimum. One that houses the "Sign-up" form and a second that confirms a successful form was completed. This is also often called the "Thank You" page. Go ahead and select "Sign up" first.
  6. Customize your form. You can edit the URL, form name, form title, and form fields. Make sure it is specific to your business and the information you are trying to capture for potential clients or customers.
  7. Next, toggle to the main Interfaces screen and select the "Thank You" page.
  8. Go ahead and customize this page based on your business and what you'd like to say to potential clients or customers who successfully complete a form.
  9. Contact information that's captured using forms in Zapier Interfaces will be stored in the Tables area. You can find this on the main Zapier Interface page, below your Pages. Zapier Tables are similar to Microsoft Excel or Google Sheets and your customers' contact information will exist in a spreadsheet format.
  10. Now, you will connect the Zapier Form to Go back to the main Zapier homepage. In the upper left corner, click on "Create Zap."
  11. Name your Zap. An example could be "Connect Zapier Forms to"
  12. For the Trigger, type in "Interfaces by Zapier" and click on the option that appears in the results.
  13. Under "Events," select when you want the action to take place. For example, this could be when a new form fill is completed.
  14. Select Continue and then select the Account, Project, and Page where this form belongs.
  15. Select "Continue" and run a test from a sample form fill.
  16. Move on to your Action step.
  17. In the App event search bar, type in "" Click the option with the orange logo.
  18. For the trigger Event, click "Call Someone (Outreach Campaigns Only)."
  19. Connect your Zapier account to your account. You'll need to add your API key when prompted. Further instructions on how to connect to Zapier can be found here.
  20. Select "Yes, Continue" after inserting your API Key.
  21. Select your account from the dropdown and hit Continue again.
  22. Under Set up action, map your fields by pairing data with what is being sent from Interfaces by Zapier. This could include first name, last name, phone number, etc.

    3. The agents will follow your campaign details for calling directions.
  23. Once you have confirmed all of the Zap details, turn it on.

All Zapier form fills will now be called back by and logged in your account. Your customers' contact information will also exist in your Zapier Tables.

How to Connect to Virtual Receptionists

If you subscribe to Virtual Receptionists, you can set up a Zap in Zapier to trigger a call for this separate service. In the "Choose app and events" step, instead of selecting "Call Someone (Outreach Campaigns Only)," you'll select "Call Someone (Virtual Receptionists)." Follow the same proceeding steps as above, save the Zap, and turn it on.
